Central Registration

  

Outreach Services (OS) offers a variety of programs in support of military families and organizations. Please download our welcome Packet.

Short Term Alternate Child Care (STACC), Volunteer Child Care in a Unit Setting (VCCUS) and Special Openings (SO) sessions are available by arrangement. These programs are designed to provide additional childcare spaces when Hourly Care and Family Child Care homes are unable to handle the demand during normal business hours. The programs are also able to provide care during non-traditional hours for events such as deployment briefings, military balls, and conferences.

OS manages all of the care for Special Needs children enrolling for CYS programs and is able to work with relocating families of special needs children to help them make a seamless transition to their new installation.

The membership of the CYS Parent and Youth Advisory Support Council (PYAC) includes all parents of children who are registered members of Child and Youth Services. Parents and staff work together to evaluate and improve CYS programs at Fort Wainwright. The PYAC, along with CYS management staff, plan parent involvement activities and explore new ways to meet the needs of the community. All CYS parents are invited to attend the monthly PYAC meetings. OS provides Babysitter Training Courses for youth ages 13-18. If they desire, teens who complete the course may have their names and availability information placed on the referral list maintained by OS.

OS provides a centralized point of contact for volunteers within the CYS delivery system. Volunteer registration, referrals and training are coordinated through the OS office. If you are interested in exploring and learning more about available CYS volunteer opportunities, contact the OS office at 907-361-7713. Background checks are required for all volunteers. Children must be registered for all CYS programs and services.

 



CENTRAL ENROLLMENT REGISTRATION PROCESS


Parents that wish to have their child/children participate in any of the Child & Youth Services programs must register in the central registration system.  There is an annual registration fee of $18 for one child, $36 for two children and $40 for a family of three or more children.  These fees are currently being paid for by Army Family Covenant.

Parents will be provided care and placed on the waitlist based upon the date that childcare is requested and their eligibility priority rating. 

Central registration provides the parent with copies of FWA CDS Request for Care Record and DA Form 7625-1 (Special Needs) forms.  Completion of these forms will allow the child/children to be placed on the waitlist.  (There is no charge to have your child placed on the waitlist).  Parents will receive the registration packet at this time.  Registration packets consist of the following forms:

·         Child Care Food Program - Application for Free/Reduced Price Meals (Form 05-06-008)

·         DD Form 2652 (Application For Department of Defense Child Care Fees)

·         DA Form 5223-R (Child and Youth Service Health Assessment/Sports Physical Form)

·         Infant Sleep Form

These forms will need to be completed in order to utilize CYS services.  Parents MUST call every 30 days to update their status on the waitlist, or they will be removed from the system.

Please note that parents of children with special needs will be provided instructions regarding the Special Needs Assessment Program(SNAP).  A copy of the DA Form 7625-1 is sent to the Community Health Nurse at Basset Hospital.  (The registration process can NOT be completed until the SNAP paperwork is provided to CER  by the CHN stating the outcome of the child’s SNAP and if we can meet the child’s needs).  SNAPs must be renewed yearly.

 

How does the enrollment process work?  When do I get care?

  1. Each program provides information on the open slot in their program to CER. Central Registration will print the wait list report.
  2. Once a child’s name reaches the top of the list, CER will call the parents to offer the designated slot.  CER will make three attempts to contact a family before moving onto the next name on the list.  When a parent is offered a child care slot- they have 24 hours to respond.  Once they accept the slot, they have 72 hours in which to complete the registration process and pay their $50 holding fee.  Billing for the child care slot will begin the first day that care is available for the child after the 72 hour registration period, regardless of whether the child is in attendance or not.  The $50 fee is non refundable once paid, but will be applied towards the first billing cycle. The parents must attend a mandatory program orientation prior to beginning care.
  3. Once ALL paperwork is complete, CER will give the parent a “Good to Go Pass” which will let the program Director know that the child is ready to begin care.  A copy of the child’s pass and their file will be sent to the program.

 

The Registration Process

Once you are ready to finish the registration process, bring your completed packet with the following supporting documentation to CER:

·         Current immunization records (include PPD and Hepatitis A series)

·         Parent ID

·         Social Security Numbers for both sponsor and dependents

·         3 Local Emergency Contacts

·         Leave and Earning Statement (LES) for both parents (if applicable)

·         Parents have 30 days to provide Central Enrollment Registration (CER) with a physical for each child

·         Single and/or Dual Military must provide a Family Care Plan within 30 days of registration

·         Guardians must provide legal documentation

(Please note that Sports physicals must be completed and current at the time of all sports registrations. These physicals must be completed yearly.  Health Assessments are good for three years at a time. All physicals and health assessments must be signed, dated and have the physician’s signature and stamp in order to be valid.) 

You may come in or call for an appointment.  Appointments are scheduled from 0800-1630, Monday thru Friday.  Appointments generally last 20-30minutes.  At your appointment, all of your household information is entered into the Child and Youth Management System (CYMS) including child shots, income information, emergency contacts (local only) home address, phone number, unit information, etc.

Once all information has been entered into CYMS CER will print 3 Medical Consent Statements and a Liability Waiver (one copy of each is send to the program).  Parents will then sign their contracts for care.  Parents will receive a Suspense form that notes all documents still needed for their child’s file.  Failure to meet suspense dates can result in immediate termination of CYS services.  Once the registration is completed, a pass will be issued for each child or youth. Registration fees are due at the time of enrollment.  CER will then send a copy of the Health Assessment, USDA form, Special Needs, Medical Consent, Liability Waiver, SNAP, Program Contract and Swipe Card to the appropriate program. 

Parents are required to update their income information with CER within 30 days of any change.  Every year, parents must come to CER to renew their registration.  At this time, they will review their child’s file and provide any required updates.  Families that have SNAPs or Family Care Plans will need to provide new ones at this time.

REQUEST FOR CARE

If you would like to request care for your child, please fill out the REQUEST FOR CARE FORM.  You may drop the form off at the office or fax to (907) 361-4198.  Please call us to confirm receipt.  You do not have to be registered to request care.  Projecting ahead will hopefully reduce your waitlist time.  If you are PCSing to Fort Wainwright you may do the same with a copy of your orders.
